The BowTieXP® Methodology

The identification of non-compliance and "potentially" non-controlled safety events (even those not subjected to Mandatory Reporting), can expose any organization to unnecessary risks. The added value for the organization is to have an accurate full report in less than an hour using "The Investigator's Black Box®", converting errors and non-conformities in valuable safety information. "The Investigator 3®" software tool includes either a "Root Cause or Tripod analysis" as part of the final report.”

As per aviation, healthcare is a high risk environment where the human error cannot be avoided, but managed.

To achieve that, it is needed the implementation of an excellent leadership, clear and well communicated policies and procedures, involving and motivating the staff, which works in an encouraging positive culture environment; supported by an effective and efficient Hazard Identification and Risk Assessment and a honest approach to investigation of safety events.
